Well, I've filled it out. What's missing in my case is making the
Summary and Details available to caregivers, as well as the ability to
give othere Read/Write capabilities.

The missing info is from notes that I typed in in earlier versions,
not imported.
 
For instance, if I went for an EKG in 2006, in the notes field I would
write the results, and I'd more...
